Jail leaders say 'human error' led to Mount Olive escape attempt

CHARLESTON, West Virginia -

At a legislative oversight committee hearing Monday, Corrections Commissioner Jim Rubenstein said mistakes during an evening head count led to an attempted escape at the Mount Olive facility on July 17. 

The three attempted escapees did not get to the outer wall, Rubenstein said. 

William Lowe, Daniel Smith and Stephen Wilson were found just after 11:30 p.m. in an area of the jail inmates are not allowed. 

Punitive action could be taken against guards, Rubenstein testified. 

Lowe was convicted of first degree murder in connection to a shooting at a Jefferson motel in 2005. Wilson and Smith were convicted for a 2008 robbery and murder.
