UPDATE:

13 News received health reports for that Pizza Hut location under the Freedom of Information Act.  Links to the full reports are posted below.

Documents we have received include health inspection reports dating back to 2010, correspondence between the Mingo County Health Department and Pizza Hut Corporation and a letter explaining the situation which shut down the location for good.

In the documents health inspectors listed several violations, some that were repeat offensives.  On almost every report an employee failed to have the proper food handler card, a requirement for handling food.

According to the Mingo County Health Department, those with direct contact of food must attend a class every 2 years and pay a fee of $10 to receive this card.  On the odd year these employees can renew their card in person or on mail.

UPDATE:

Pizza Hut Corporation released a statement on Tuesday, Feb. 25, about their store in Kermit, WV.

"I can confirm that the store location will be closed permanently. It's not the kind of experience we want for our customers in Kermit."

UPDATE:

On Tuesday, 13 News uncovered shocking surveillance video from inside a Mingo County, WV Pizza Hut. The video, recorded several weeks ago, shows a district manager caught urinating into the restaurant's kitchen sink.

Concerns over the video have continued to circulate around the community of Kermit, where the restaurant is located.

"I just don't trust anybody after it happened once, plain and simple," said Ricky Brewer who lives nearby the now temporarily closed restaurant.

"I would never go there and eat," said Blanche Ferris. "Even if I did, the thought would run through my mind."

Mingo County's Sanitarian Brett Vance closed the restaurant Tuesday morning after watching the video. He said it will be temporarily closed until further notice.

Pizza Hut Incorporated has confirmed the man in the video was the District Manager and has since been fired. In an email to 13 News, a representative from Pizza Hut Incorporated said the Kermit location "did not have previous problems reported. Meaning, it was a store in good standing."

A cleaning crew is flying in from Texas to clean and sanitize the Kermit location as well as the other restaurants the District Manger was in charge of, including Pizza Huts in Ceredo, Danville, Chapmanville and Logan.

All of those locations are franchised by Rage Inc. 13 News reached out to Rage Inc. for the second day in a row. On Tuesday they did not return our call. On Wednesday they referred us to Pizza Hut Corporate.

"If it was cleaned, washed, fixed up I wouldn't care a bit to eat there," said Charles Jude, who said he was a "regular" at the restaurant.

Officials with the Mingo County Health Department sat down with a representative from Pizza Hut Corporation Wednesday to discuss future plans of action.

UPDATE:

Inside of the Pizza Hut in Kermit, West Virginia surveillance cameras captured one of your worst fears coming to life.

"Oh my god, he just peed into the sink," said Tracy Johnson of Mingo County, while she watched the surveillance video.

The video recorded a few weeks ago shows a man walking over to the deep sink and urinating in it. The same sink used to wash kitchen utensils and supplies. The Pizza Hut Corporation has confirmed the man in the video is a Pizza Hut District Manager.

"Made me sick out of my stomach to think that I ever ate there and that happened," said Johnson. "But I will never go there and eat again."

Michelle Salmons of Kermit and her family wanted to eat at the restaurant. However, when they pulled up the doors were locked.

13 News showed her the video and she said, "That's disgusting. That is just horrible. Oh my goodness and we eat there all the time."

13 News contacted the Mingo County Health Department regarding this video. Brett Vance, the city's sanitarian, watched the video and went to the restaurant to temporarily close it.

Now a sign hangs on Pizza Hut's front door saying the restaurant has been shut down because of "conditions within the establishment constituting a substantial hazard to the public health."

"It's something you hope to never have to deal with but we take it seriously," he said. "They're closed until we do a thorough cleaning on the place and have the reassurance there are safeguards in place."

The Pizza Hut restaurant is franchised by Rage Inc. out of Wichita, Kansas. 13 News reached out to Rage Inc. and the Pizza Hut Corporation about this video.

In a written statement Pizza Hut said:

"First of all, we are embarrassed by the actions of this individual. Pizza Hut has zero tolerance for violations of our operating standards, and the local owner of the restaurant took immediate action and terminated the employee involved. While the isolated incident occurred during non-business hours and did not involve any food tampering, we follow strict safety and handling procedures and the restaurant has since been closed. We apologize to our customers of Kermit, West Virginia and those in our system who have been let down by this situation."

Mingo County Health Department officials said it could be days and even weeks before they consider reopening the Pizza Hut.

Rage Inc. operates six Pizza Hut locations in West Virginia. The restaurants are located in Credo, Chapmanville, Danville, two in Logan and Kermit.
